Making An Appointment
We welcome you making an appointment by phoning our office on (07) 5564 8011 during our office hours.
Appointments can be requested online, and our office will contact you to confirm your appointment times. Please note, an online appointment request does not mean you have a confirmed appointment.
A referral is essential and enables much of the cost of treatment to be covered by Medicare.
If you are wanting an appointment for fertility management and have a partner, they will also require a valid referral.
A standard referral from your General Practitioner (GP) is valid for 12 months. A referral from a Specialist is only valid for 3 months.
New Patient Registration Form
If you are a new patient to our practice, we look forward to welcoming you at your confirmed appointment.
A New Patient Registration Form can be downloaded and completed prior to your appointment. Alternatively, you can fill it out when you arrive at your first appointment. Providing as much detail as possible assists us to understanding your requirements.
You should also bring to your appointment your referral letter and relevant results or scans.
Existing Patients
If you are an existing patient of Dr Flynn or Dr Allanach and your personal details, or your referring doctor have changed, you will need to complete a new patient registration form, which we will add to your files.
Ask your GP for a referral to Dr Michael Flynn - print this Referral Request and hand it to your GP.
